Nature of Services
Forward Momentum provides health and wellness coaching services. Coaching is a collaborative relationship focused on helping clients identify goals and create sustainable lifestyle changes. Sessions are conducted virtually via video call.
Not Medical Advice
Health coaching is not a substitute for professional medical, psychological, or nutritional advice, diagnosis, or treatment. The coach does not prescribe, diagnose, treat, or cure any physical or mental health condition. Clients should consult their physician before making dietary changes, using supplements, or discontinuing any prescribed medication. Any information provided during coaching sessions is for educational and motivational purposes only.
Student Coach Disclosure
Leah Horgan is currently enrolled in an NBHWC-approved health coach training program and is not yet a fully certified coach. Coaching services are provided for educational and training purposes. Clients acknowledge that coaching skills and knowledge are still in development and are solely responsible for their own decisions, actions, and results.
Personal Responsibility
Clients take full responsibility for their own health, wellbeing, and decisions made during and after the coaching relationship. By engaging in coaching services, clients expressly assume all risks associated with lifestyle changes, including the introduction of new foods or wellness practices.
Scheduling and Cancellations
Sessions are 30 minutes in length. If you need to cancel or reschedule, please do so at least 24 hours in advance. Late arrivals cannot be accommodated beyond the scheduled session end time.
Limitation of Liability
To the maximum extent permitted by law, Leah Horgan and Forward Momentum shall not be liable for any direct, indirect, incidental, or consequential damages arising from your participation in coaching services. The sole remedy available in the event of a dispute is a refund of coaching fees paid.
Dispute Resolution
Any dispute arising from these services shall be resolved through binding arbitration in accordance with the American Arbitration Association rules. This agreement shall be governed by applicable state law.
