Nature of services
Forward Momentum Health & Wellness LLC provides health and wellness coaching services. Coaching is a collaborative relationship focused on helping clients identify goals and create sustainable lifestyle changes. Sessions are conducted virtually via video call. Coaching services are educational and supportive in nature and are not healthcare, psychotherapy, counseling, medical treatment, nutrition therapy, legal, or financial services.
Not medical advice
Health coaching is not a substitute for professional medical, psychological, or nutritional advice, diagnosis, or treatment. The coach does not prescribe, diagnose, treat, or cure any physical or mental health condition. Clients should consult their physician before making dietary changes, using supplements, or discontinuing any prescribed medication. Any information provided during coaching sessions is for educational and motivational purposes only. No physician-patient, therapist-patient, dietitian-client, or other licensed professional relationship is created through use of this website or coaching services.
Personal responsibility
Clients take full responsibility for their own health, wellbeing, and decisions made during and after the coaching relationship. You acknowledge that all decisions regarding your health, wellness, lifestyle, exercise, nutrition, supplementation, and personal goals are your sole responsibility and are undertaken at your own discretion and risk.
Scheduling and cancellations
Sessions are 45 minutes in length. If you need to cancel or reschedule, please do so at least 24 hours in advance. Late arrivals cannot be accommodated beyond the scheduled session end time. Missed sessions or cancellations made with less than 24 hours' notice may be forfeited and are not eligible for refund unless otherwise determined by Forward Momentum Health & Wellness LLC in its sole discretion.
Limitation of liability
To the fullest extent permitted by law, neither Forward Momentum Health & Wellness LLC nor Leah Horgan shall be liable for any indirect, incidental, consequential, special, exemplary, or punitive damages arising from or relating to the coaching services or use of this website. If liability is established notwithstanding the foregoing, the total cumulative liability of Forward Momentum Health & Wellness LLC and Leah Horgan shall not exceed the amount paid by the client for coaching services during the twelve (12) months immediately preceding the event giving rise to the claim.
Dispute resolution
Any dispute arising from these services shall be resolved through binding arbitration in accordance with the American Arbitration Association rules. This agreement shall be governed by applicable state law.
No guarantees
Coaching outcomes vary from person to person. Forward Momentum Health & Wellness LLC makes no representations, warranties, or guarantees regarding any specific health, wellness, weight-loss, behavioral, personal, professional, or financial outcome.
Intellectual property
All content on this website, including text, graphics, logos, downloads, videos, worksheets, and other materials, is the property of Forward Momentum Health & Wellness LLC and may not be copied, reproduced, distributed, or used without prior written permission.
Eligibility
This website and coaching services are intended for individuals eighteen (18) years of age or older.
Changes to terms
Forward Momentum Health & Wellness LLC reserves the right to modify these Terms at any time. Updated versions will be posted on this website with a revised effective date.
